About
Jordan Health – Brown Square Community Health Center, located at 322 Lake Ave in Rochester, New York, stands as a cornerstone of community wellness and a beacon of accessible, comprehensive healthcare in the Greater Rochester area. Formerly known as the Anthony L. Jordan Health Center, the organization traces its roots back to 1967 when it was founded as the North East Area Development (NEAD) Health Center. This genesis was born from the civil rights movement and a pressing community need for equitable healthcare services, particularly for the underserved African American population on Rochester's northeast side. Today, as a Federally Qualified Health Center (FQHC), Jordan Health has evolved into a multi-site network, with the Brown Square location serving as a vital access point, embodying a decades-long commitment to its founding principle: 'Healthcare is a right, not a privilege.' The center operates under a mission to provide comprehensive, high-quality healthcare services in an environment that embraces dignity and respect for all, regardless of their ability to pay. Its vision is to be the premier, recognized patient-centered medical home and health partner for the communities it serves. What sets Jordan Health – Brown Square apart is its deeply integrated, patient-centered model that goes beyond transactional medicine. It functions as a true medical home, offering a vast array of services under one roof, from primary and pediatric care to behavioral health, dental, and pharmacy services. This integrated approach ensures continuity of care, improves health outcomes, and reduces barriers for patients managing multiple conditions. The center's expertise is particularly noted in serving vulnerable populations, including low-income families, uninsured or underinsured individuals, and communities of color, addressing not just clinical needs but also the social determinants of health through care coordination and community partnerships. Its unique value proposition lies in its unwavering commitment to accessibility, cultural competency, and holistic care. As an FQHC, it operates on a sliding fee scale based on income and family size, fundamentally removing cost as an insurmountable barrier to health. The staff is trained to provide care that is linguistically and culturally appropriate, fostering trust and understanding in a diverse patient population. Furthermore, Jordan Health distinguishes itself through its active role as a community anchor institution, engaging in outreach, health education, and partnerships with local organizations to tackle broader issues like housing, nutrition, and employment that impact community health. In a healthcare landscape often fragmented by specialty and finance, Jordan Health – Brown Square Community Health Center stands out as a unified, compassionate, and resilient institution dedicated to healing, empowering, and strengthening the Rochester community, one patient at a time.

Customer Reviews
Great people specially Raquel, she is very helpful. She has been such a big help to me and my family, and we’re so grateful for everything she has given us. Whether it’s baby food, clothes, or other things we’ve needed, she always shows up and give from the heart. Her generosity has made a real difference in our lives, and it means more than words can say. She has helped us through tough times, and we’ll never forget her kindness. We thank her for always thinking of us and for being such a giving and thoughtful person.
I have been here since I was a baby my child go here I love how they really treat you like family especially my care manager Raquel she has been such amazing person thought my pregnancy and after she has helped me so much I don’t kno what I would have done without her from appointment scheduling to just being a ear to Listen to my problems, offering me resources ,she genuinely is a great person who makes me and all her patients feel important and like where not alone, she cares so much bless her big heart and brown square and also me are so lucky to have her
Everything when I went to The dental office was really good. The people were really nice and 1 of the dentist there really went in depth about what's wrong. Everyone was really helpful. The wait time is pretty long though. But my experience there was amazing and were informative. Just always make sure too ask questions.
The receptionist was rude upon my arrival. She interrupted my first appointment, and provided incorrect information in regards to my insurance, advising me it has been canceled but another application was being processed. This was incorrect as my insurance is active until September. Upon calling my insurance company they verified my coverage (including dental) was active, and they also verified they had no record of her calling to verify my coverage. She was extremely unprofessional, rolling her eyes, sighing, and preforming other dramatic gestures to let me know how unhappy she was to be doing her job, and trying to be argumentative when I was just trying to explain the information. Never have I been treated so poorly when trying to seek help from a medical facility. I can’t imagine how self pay people are treated, just based on my interactions with her and thinking I didn’t have insurance. People without insurance deserve dignity in treatment.
unprofessional, don’t ask for a number to complain to . They will just kick you out and say you’re threatening them. When in all reality they don’t do their jobs. Why is it so hard to make an appointment while both calling and coming in , and why am i not able to talk to my doctor in person or via email? Because people are lazy not doing their jobs. Please hire new staff asap. Reviews are at 2 stars and that is ridiculous.
